Dave Reid's recent accident got me to look more deeply into something I'd heard about a while back: Divers' evacuation insurance. The organization is DAN: Divers' Alert Network. http://www.diversalertnetwork.org" onclick="window.open(this.href);return false;, a non-profit outfit. One benefit of membership is evacuation insurance (medical only, no rescue because you got your boat stuck) that covers up to $100,00, just about anywhere. You do have to call them first. Anyway, their website has all the details, look at membership handbook.
No, I'm not getting a kick back, just thought some of you would be interested. They also offer other travel insurance that does cover equipment and lots of other stuff (dependants, spouses, vehicles, etc). I guess this would necessitate having a sat phone in many instances to reach them. More toys.

I started using them several years ago. They are the standard with PADI. They take all the research out on what facility and dr. to see in the event you need one. Their plan works all over the world and will tell you where the best medical facilities are near your dive site. I also carry the rescue insurance with GEOS SAR. It's all very cheap considering that accidents can be very costly, especially if out of the country and then the addition of a SAR.

We carry the SPOT almost everywhere. We haven't bought the insurance yet but when things get more remote, we will.
Amazing how much better you feel on a river knowing that help is reachable if needed. A bonus is the check-in feature that lets loved ones know if you are ok. It also has 2 levels of emergency: help and 911.

We also have carried the diver's evac insurance whenever we have a major Western raft trip. Never used it but should have when I tore my shin open on the Selway in '09. I followed up later and inquired with the company how serious does an injury have to be to be covered. The injury HAS to happen in the water. I didn't get a specific answer on how serious it has to be, except serious enough to require evacuation. But the cost is certainly worth it, compared to the cost of evacuation. Seems I remember the NPS (in GC) part doesn't cost the evacuee, but the shuttle from their pad to Flag or wherever does. Someone else might weigh in on this.
