I'm perplexed.

Are they saying no gravel bar campfires, or gravel bar campfires allowed when contained (e.g. firepan)? The "except as permitted above" caveat is the source of my confusion.

Seems clear to me.
Unless there is a provided metal ring or pedestal grill. No fires in the Wild or on gravel bars. Period. Fires are only allowed in developed areas which provide the afore mentioned appliances (i.e. provided metal rings or pedestal grills)
Maybe best to contact the NPS for further clarification if this seems unclear to you.
